Online Reimbursement Form: To be used for reimbursement on expenses. The PTO does not reimburse for taxes. To ensure you aren't charged for taxes, print a copy of the Tax Exemption Certificate and turn it in to the business you are purchasing from. All reimbursement forms must be accompanied by the sales receipts. These can be sent electronically to Treasurer, Mary Labatt.
**If you have a large invoice for a standard Cambridge project such as Mule Market, Mule Games and Balloonfest, etc., our PTO Treasurer can pay the invoice for you instead of you paying first and being reimbursed. Remind the vendor that we are tax-exempt.
Cambridge PTO Deposit Form: To be used to deposit checks or cash collected on behalf of the Cambridge PTO. Please print, fill out check numbers or cash deposited, and turned in to Treasurer's Mailbox in the office. Deposits less than $100 should be dropped off in the PTO Mailbox in the Cambridge Foyer. Deposits $100 and larger are to be handed directly to office staff members Gina De La Cruz or Jenny Gonzalez to go in the campus safe. If you have turned in a deposit manually, please email PTO Treasurer, Mary Labatt.
Tax Exemption Certificate: The Cambridge PTO cannot reimburse for sales tax as we are a tax-exempt organization. When you are purchasing something for the Cambridge PTO, you may present the retailer with our tax-exempt certificate. If you pay sales tax, you will not be reimbursed, so please remember to have this certificate with you.
